If your server is running Linux, then you should absolutely NEVER reboot it, unless it is crashed, or there is a kernel upgrade. There's no reason to reboot a Unix system. Worst case, you could restart all daemons if you need to (like after a timezone change), but that is it.

Once a month is good. But not necessary if you do not have a issue with the system. ( If using Linux )

I haven't really rebooted mine for 4 months now o.o never felt the necessity to do so.

Mine gets rebooted when the host upgraded the kernel on the controlling node. That's been twice in 18 months.

If its a test box then with some frequency as I play around with things other then that like others have said (kernel only).
