~ 2-4 TB hdd space in raid 10 or 5 (even software raid)
~ 4 GB RAM
~ 2+ cores modern CPU
~ $200 per month

I ready to pay big setup fee.
Please suggest.

I'm sure there are any number of providers who can do that for you. Your budget and specs look reasonable. Is there a specific amount of bandwidth you'd be needing? That would be helpful to know when suggesting a provider. Many providers should be able to do the above, $200 / mo, without a setup fee, if the bandwidth needs aren't too high, and many providers, even giving you a lot of bandwidth, could do it with a small setup fee.

I sure it reasonable requires for such price.
But I really troubled no one big hosting company offers such price options in pricelists...

you might have to contact a sales department, but there are certainly places who can offer core2duo (or better), 4 drive raid 10 (2-4tb space available), 4gb ram, and a few tb a month b/w for $200 even without a setup fee. The softlayers of the world, maybe not, but many reputable and reliable providers could do this for you if you contact their sales department.

It's worth keeping in mind, that once a hosting company has a customer at a particular price, they will be very hesitant to update their home page standard pricing to be lower than what their existing customers are paying, lest the existing customers ask to have reduced prices. So often in this industry, the home page pricing will stay the same for several years, even as the prices that the sales department is willing to give you drop lower and lower.

As others have said, HostDime and Limestone Networks should do. Many companies who don't offer a large hard drive on their website, probably can offer it if you contact their sales department. In that case, you really can choose most of the companies in the offers section and ask their sales department what they can do. Maybe you can even get a discount from some of them.

I look all suggested sites and found no one good enough.
No one offer buydown price for extra hardware. A lot of strange (reseller?) sites with no datacenter description.

You could check Netdepot. We have been using them for couple of years without any problems and they have buydown price for monthly fee as well as for hardware components.

I know with Hurricane Electric, it is pretty much a buydown. Last Time I had a quote, it was something like $100 a month and $500 setup fee for a basic server.
