Inconsistency on the cPanel database, might have to re-create the account.

Login as WHM (Root) and check the master PhpMyAdmin for a database by with that username.

The database might be deleted from /var/lib/mysql but there are traces left on the system

if you don ot have data on that db try to remove and re create another one. Make sure that if you have created the mysql db otherthan by using cpanel interfact it should use the standard format

cpusername_db name otherwise it wont be showed in cpanel
