I found OTRS to be much difficult to use (user interface) than OS Ticket.

If you are looking for a solid system then i strongly advise that you spend a little money, Kayako eSupport (leading support system) can be had for $25 a month which in the grand scheme of things is not that much, also if it is for a hosting biz then your billing system will most likely provide a support section, we currently run WHMCS and find that the built in helpdesk is fantastic
If you need help about anything to do with WHT, check out the Helpdesk

I know the thread is asking for the best free solution but I must agree something like this might be worth investing a little money. I started with Ubersmith Lite and upgraded to Ubersmith Pro hosted at there location (To keep communication open in case of network issues). I just recently upgraded to Ubersmith DE. The entire process was cheap starting out and was very easy to upgrade and migrate since I started with the software I was going to use as my main solution. If starting my business over there might be some things I did a little different but this would definitely NOT be one of them. I would think heavily on something like this especially if your planning to move or migrate later. One advantage I had as well is the customer never noticed a major change in system just added benefits along the way.

Just installed Trellis Desk for testing, and its simple, clean and does the trick.
Had some problems with the email piping, but a quick edit in a pipe.php and a quick pear mail install did the trick.

Got a Kayako full with no branding laying around, cant decide if i want to use it or not.. Trellis might be enough for my new project.

Trellis is very user friendly ticketing system. One more vote for Trellis.
