Are you putting these field content into an SQL database? If so you will need to connect to your sql server, and then an INSERT statement with $_POST['name'], $_POST['email'], $_POST['description'].
For instance, here is the code I would use if I needed to put it into a mysql database.
// It's a good idea to sanatize the values before using it to prevent sql injection attacks
$name = mysql_escape_string($_POST['name']);
$email = mysql_escape_string($_POST['email']);
$description = mysql_escape_string($_POST['description']);
// Build your insert query
$query = "INSERT INTO table (name, email, description) VALUES ('$name', '$email', '$description')";
// Then execute it.
