rfq for order form

I need a quote for converting the order form located at
to php and or incorporating it into oscommerce.

the current form uses html and 2 .js files I'll be happy to pass those along if need be in order to get an accurate quote.

It would be nice if the forms values could be easily changed as they have 3 forms that are region specific and hence the pricing and such is specific to each of the 3 regions as well.
