Pardon if I misunderstood that this section role is as "offbeat".

If you can input your suggestions about the VoIP providers within the North-East USA, if possible the most reliable ones with their servers in NY/NJ/CT, with an reliable latency, ping reply under 50 / low TTL under 50. I am looking (mostly) for inputs from NY EarthLink/RR/Optimum/RCN customers.

Not sure if this is exactly what you're after, but ...

I use callcentric.com who have servers in NY. I also run asterisk of a VPS from lylix.net and the ping between the two is around 2ms.
