A mass defacement


Hi everyone,
I have a problem I have a lot of sites that have a defacement.
In all hacked sites there's an iframe into it that point to a site with a trojan hourse.
In my server I found a lot of sites with this problem, I'm worry about this because a lot o f sites with only the skeleton page .html was hacked, so sites without any php scripts are hacked.
Is the server security compromised ?
Can you seggest me something?

yeah most likely server was compromised
