    I've been using les.net for a little while now. They provide toll-free DID termination.

    Their rates are decent, and Les is always around via phone/email if you have a question/problem.

    800 numbers

    We have been using kall8 for about 3 years now never had a problem. The rates are not bad.

    We have also been using ringcentral. We recommend them as well.

    If I had time I would set it up to run on asterisk (trixbox) to have a full pbx system in place.

    We have been using RingCentral (http://ringcentral.com) for quite some time now and can't say anything bad about the service they provide other than they only have telephone support during business hours (Not that bad) -- All of their plans come with the basic features plus the ones they develop and when they do they simply add it onto your account with no action required by the client. I highly recommend them.

    kall8.com - highest levels of flexibility (routing, voicemail, ability to block callers by number, state and even country) + very low rates - like $2/month + time (at AT&T they charge $40/month per line + twice more for minutes).
