    Do you need the data added to a database or just emailed to you? Do you require a billing system to charge users once they fill the order form (you will need a more advanced system for this)?

    Name
    Address
    City
    State/Province
    Zip/Postal code
    Country
    Phone
    Email address
    MSN/AOL

    ITEM 1
    ITEM 2
    ITEM 3

    i want to get all this info from the user and then they will pick one of the item. For example if they pick item 1. and they submit that info... it should send me all the users input as well should go to paypal to get the payment. so on the submit button it should have to function running one of them is send users input to my email as well go to paypal to make the payment? AS i want to use php but is there any script that you guys recommend?

    THANKS ALOT FOR YOUR HELPS GUYS. THIS REALLY HELP ME TO GET MY BUSINESS RUNNING.

    Well in that case let me see if I can come up with fast some code. You should have 2 files, signup.php and signup2.php (at least that's how I do it, makes the code a bit clearer).
    here is signup.php

    Code:
    <table cellspacing="0" cellpadding="2" width="400px" align="center">
    <form action="signup2.php" method="post">
    <tr>
    <td colspan="2"><b>Customer Information</b><br></td>
    </tr>
    <tr >
    <td >Name</td>
    <td><input type="text" name="name" size="30" value=""></td>
    </tr>
    <tr >
    <td >Address</td>
    <td><input  type="text" name="addr" size="30" value=""></td>
    </tr>
    <tr >
    <td >City</td>
    <td><input  type="text" name="city" size="30"  value=""></td>
    </tr>
    <tr>
    <td >State</td>
    <td><input  type="text" name="state"  size="30" value=""></td>
    </tr>
    <tr >
    <td >Zip</td>
    <td><input  type="text" name="zip" size="30" value=""></td>
    </tr>
    <tr >
    <td >Country</td>
    <td><input  type="text" name="country" size="30" value=""></td>
    </tr>
    <tr >
    <td >Phone</td>
    <td><input  type="text" name="phone" size="30" value=""></td>
    </tr>
    <tr >
    <td >Email</td>
    <td><input  type="text" name="email" size="30" value=""></td>
    </tr>
    <tr >
    <td >AOL/MSN</td>
    <td><input  type="text" name="messenger" size="30" value=""></td>
    </tr>
     <tr>
    <td >Hosting Package</td>
    <td><select name=package>
    <option value="" selected>(please select your package)</option>
    <option value=1>Package1 </option>
    <option value=2>Package2 </option>
    <option value=3>Package3</option>
    </select></td>
    </tr><input  type="hidden" name="add" value="1">
    <tr >
    <td colspan="2" ><input name="Submit" type="submit"  value="Submit"></td>
    </tr>
    </form>
    </table>
    and signup2.php

    PHP Code:
    <?php
    if ($add==1)
    {
    $message '
    New Customer info \n
    Name : '
    .$name.' \n
    Address : '
    .$addr.' \n
    City : '
    .$city.' \n
    State : '
    .$state.' \n
    Zip : '
    .$zip.' \n
    Country : '
    .$country.' \n
    Phone : '
    .$phone.' \n
    Email : '
    .$email.' \n
    AOL/MSN : '
    .$messenger.' \n
    Hosting Package : '
    .$package.' \n
    '
    ;
    mail("[email protected]"$message "New client!");
        if(
    $package==1){
        echo
    'Pay via paypal:<br><br>';
        echo
    'paypal code for product1 goes here';
                       }
        if(
    $package==2){
        echo
    'Pay via paypal:<br><br>';
        echo
    'paypal code for product2 goes here';
                       }
        if(
    $package==3){
        echo
    'Pay via paypal:<br><br>';
        echo
    'paypal code for product3 goes here';
                       }
    }
    Just insert your paypal code where is says paypal code for product3 goes here.
    Change the email and it should work just fine.
    Once again this is a bit dirty so you should play with it - change tables, colors fonts etc, verify all the data via javascript or php etc but it should be a good start. All the best.

    Well I haven't closed the php (silly me). Just put ?> at the end of the second file. You might have a bit of problem with the paypal code if it contains a ' symbol. If so just remove it (it's usually in the sentence "Make payments with paypal. It's fast and easy").
