Other than a couple of places it validates as HTML 4.01 transitional. The places it doesn't validate are, if I recall where I wanted to use a bold or an anchor in a line item or that sort of stuff.
The PHP included would be good for someone who's never done a form and wants to see how it's done.
There's no CSS with it really, it's just embedded. The reason was I had plans on making additional sections but, in reality, it was just not looking that good.
Either way, it's up there for now if anyone wants it. You'll need to strip the copyright information out of it but consider this written permission to do so as it's now public domain as far as I'm concerned.
