I think this is what you wanted to know, but I'm not sure:
If you get a 20% chance, you'll get the 20% banner. If you answer none you get 10% and if you answer all you'll get 95%; PSD for the banner comes with it so feel free to change it.
You can change the questions very easily, they're all yes no, just change the text.
To add a new question (say you want 11 questions rather than 10) it isn't hard, just a little time consuming (about a minute or so). There isn't a control panel for the script but it shouldn't be hard with a little PHP Know how to add another question, it just involved duplicating a bit and renaming it and adding it into the addition to work out how many questions they've answered yes to (it's easier to do than explain).
If you want more questions adding I can add them for a price per question which we can arrange privatly.
