Well popular billing solutions complete with their own order forms are modernbill, whmautopilot, clientexec, and a few others. Each of the ones I mentioned come complete with an order form.

If you can afford $11.95 a month, look into ClientExec (http://www.clientexec.com) - It's an all in one billing suite and it works great.

You could also take a look at ModernBilling which I believe is a little bit more, and it does have more features then ClientExec...
