Website, logo, company identity stolen
Someone has stolen my company webpage, newly designed logo, and very company identity. I ran into http://www.kawio.com on a forum post -- it is theft of my company site and the logo I just had designed in a SitePoint.com forum contest less than a week ago. The page background image is stolen from my personal blog which went up about a week and a half ago. This is recent work.
Does anyone know how I can find who's hosting these sites and where I can find a standard DMCA takedown notice to send? I would like to proceed with that before hiring a lawyer of course. Thanks.

Get the domain owners name and contact information then call them with your complaint.
Then contact the actual hosting company, the site is hosted with buyers-heaven.com

Well, I found three good DMCA notices online for sending to service providers, and used them to write my own to send to Managed.com and Buyers Heaven. I CCed the Managed notice to Buyers Heaven and the gmail address above. I CCed the Buyers Heaven notice to the address above. Now I just need to find how to word the notice to the site owner himself. Hopefully one of those two hosting providers will stop this guy.
Weird thing: Outlook Express changed the gmail address above to a name, meaning it was in my contact list. I checked and this person e-mailed me once before, a one-liner last month, asking if I would sell awio.com. Of course I replied that my company site was not for sale. Kawio.com was registered in July, a few days after I said awio.com was not for sale, my business was formed prior.

*ahem* Anyway, I think Managed will take the appropriate action though they might ask for some kind of proof as their standard verification procedure.

Yes, I know. A bit flattering that someone would want to model their 'network' after mine. I still can't tolerate it though, especially since I just paid to have this logo designed less than a week ago.
All 3 notices sent and CCed to respective parties, hope that does it. I would hate to have to go to court, but it would be an opportunity to talk to a lawyer and I could use some advice on other things anyway

I notice kawio.com is registered at GoDaddy. Don't forget to notify them of the situation -- I'd telephone them, actually. If I know GoDaddy, they'll take immediate action and suspend his account, including all other domain names he has through them. Let us know who takes action, and in what order. Also, sounds like a kid -- cc his parents. Rippers suck.

Regarding http://digitalzero.com/hosting.php , none of the order buttons work, they all loop back to the page ==> no sales can be generated (or is it some IE-only code?).
