The OLAC usually costs $100.00 for every 20 pages. If you have more then 20 pages there will be an extra small fee.

I'm not exactly sure what you mean by template based. But basically if you wanted one, I would make it up and you'd tell me all the changes you wanted.

Would a template cost? I know some people interested in templates.

It really depends on what you want. Usually anywhere from $100-$300
