I agree with #2, but would #1 really be necessary? If people need to be logged in to use host quote (not sure if they do), then their e-mail would already be verified since it must be when they register.
I don't like it... because for one if your a host you have to pay hundreds of dollars to just be able to see the forum that the requests get sent to, and also if you filling it out you have to basically fill out every bit of information about you, and if your just looking at prices you may not want to put that much information out.
EDIT: The Money part and the information part is the bad part, if it were free for a host to use it and if you only had to put your name what your looking for and some contact information like a email, phone number, AIM/ICQ/MSN. Something like that
