123-reg.co.uk don't play with DNS information at all.
By default all new domains will have the Name Servers set to HOSTEUROPE - I believe this is the case for all new domain purchases from anywhere, they will set them to their own name servers to that "Holding Pages" work.
Once the domain is fully registered and under your control you can login and change the name servers as much as you like.
123-reg don't care what you set these too, however they do not go araound and change them back to HOSTEUROPE - why should they? What do they gain from doing so?
It sounds to me like your client has bene playing with what he doesn't know.
Under the 123reg Control Panel there is a single check box along the lines of "Transfer the domains back to our control" and by checking this you reset the Name Server information back to HOSTEUROPE.
I've been a customer of 123reg for a very long time, both for personal domains and all of our work ones are also with them.
I've got numerous friends who also use this company and never have we known an issue like this to happen.
You've got free technical support from 123reg, so give them a call.
I've got about 200 domains with them and they have never changed my DNS entries. They have had some problems with their redirect system recently. Their help desk support is OK, although a little snowed under with the volume of queries. You might think they're crap, but by comparison with a lot of the other .co.uk registrars they are fantastic - no hidden charges, freedom to make whatever changes you need using the control panel, low cost, attractive affiliate scheme, the list goes on.
This sounds more like somebody using the control panel incorrectly.
There may have been a problem with the nameservers the client was/is trying to change to. If Nominet doesn't like the new nameservers, the change will be rejected and the original nameservers will remain.

Yes, he told me that it was saying "NameServer may not exist" or something like that.
