We need an order form so that our customers can custom design their cumputers and get new price quotes. A very basic example of what we are looking for is at

It is preferable that the form is done in php if at all possible, or some code that we could make basic product name and pricing changes to ourselves (in the future).
We will need the form to have 12 drop down fields and each field will have between 2-12 options.
The form needs to update the price of the computer each time a new selection is made. Also, a more information button/text next to each field will be required.
Possibly the most important is that this form needs to work with paypal. When a customer clicks on the buy button they need to be taken to the paypal page that will have the proper price of the computer they have configured. Also, we of course will need to receive the form that they filled out via email.
If you already have a product that fits our needs that is fine, otherwise you will have to custom design this form. It should not be overly difficult.
