Looking for someone to design a php form :)


Hello. I own a loan company, and we are in the proccess of launching our new website. We want people to be able to submit a loan application online... Via a (php contact form.)... We have all the form questions etc. Here's the thing: we need to receive the application via email, and it needs to look like the actual application, not standard email form. The reason for this is non-computer-geniuses...LOL... Let me elaborate further... When we receive the application the person submitted a few minutes ago, it needs to look like a form, not a list of values. We prefer this to be written in php... Perl is okay as well.
