For a web hosting provider:
1- Knowledgeable management (it all starts from there..)
2- A Good blend of Tier 1 network providers with a route optimizer (example: noction, internap..)
3- Quality servers
4- Knowledgeable staff that will be able to manage the servers...
5- No support outsourcing (all done in house)
6- Everything (from A to Z) must be done correctly (the provider must not try to be cheap on something...)
7- Offsite backups included for the clients
8- Charge what it needs to be able to provide the service described. Don't sell a dream.

There is not a perfect one. Some need price, some location, some 24/7/365 endless support, some exact payment method/s, and a lot more. Even the perfect blend will not work for a lot of customers. Overall the perfect provider is always a mix from $1/mo, 24/7 advanced technical support, fastest server and Internet

When you say 'premium', as compared to what? Not-premium? Average?

It depends upon the number of websites/web apps you're running, and their resource consumption, and tons of other factors, but the bottom-line is that customers must know what the threshold limit really is.

Hi,
Quality hosting does not cheap, cheap does not come with quality hosting. If you can have both at the same time, you will become premium choice.

Quick email support, and telephone number where I can get hold of someone who can help me. Support that knows their tech, and can either give good suggestions or dig in deep to debug. Good reputation, and doesn't leave their clients hanging. Servers need to be lightly loaded and fast 99% of the time I'm myself accessing the site.
To me it's mostly about support, and good overall operation.

According to me, customer/s tags his/her provider as "Premium" if they get assisted properly, on time, try to sort their hosting problems as soon as possible and in a professional manner, their staff who assist people and strike a deal would be knowledgeable (at least about hosting industry), up time, competitive price. I think these are certain basic things which every clients expect from their hosting providers, which I think is quite justifiable.

I would also add - friendly customer service with personal attitude. It is extremely annoying to feel like you speak with a bot or if you get only canned responses.
