InverseFlow is easy to install, feature filled and has an intuitive interface for both the administrator and clients.

You forgot Modernbill... They offer a helpdesk built into the modernbill billing package...

I would go with perldesk, we used it for over a year then decided to go with perlbill so our billing and support could be in one package.

Cerebus's setup looks nice as well, but I still think perlbill/desk is the best all-around helpdesk.

Though, it is pretty sad that they dont even use their own helpdesk Im pretty sure they use Kayako.

We have inverseflow and deskpro. We are migrating everything to deskpro. Inverseflow is a child compared to deskpro.
