Paypal authorization form
Is it normal to be asked to fill out a paypal authorization form like this especially when I live in the US?

I've never been asked to fill anything like this before since I've ordered multiple VPS and dedicated servers and honestly I am annoyed at the host for asking me to do something like this.

No, it is not normal, based on my experience. Fortunately, there are plenty of hosts that don't ask for that or only ask for it if the order has indications of being fraudulent.

We would have only asked you to fill this form if you paid using unverified paypal account. I am sure most of the providers will do the same to avoid any issue.

Well I have a verified personal account. I just don't have another bank account to tie to my premier account.

I imagine it's for the protection of both parties involved. Although, it is not something I've seen before and I can understand why you would be concerned. But with all the fraud now days, I can't say I blame them for taking such measures.

Well what I'm wondering now is that if it is worth the hassle to make another bank account to tie to my premier account so I don't have to fill forms like this every time.

I know it seems irrelevant to you, but I am wondering how prevalent this requirement is, and if I can avoid future paper work by making another bank account to verify my premier account.
I don't want to email every provider here and ask them if they require a paypal authorization form regardless of whether it is verified or not.

At BurstNET, not everyone has to fill out the authorization form. It is only necessary if you fail the auto-fraud checks we have in place. You are also only asked for it once.
