They must be using a highly optimized Mambo with cache and db cache like memcache. I'm not sure about the server but it hosted at softlayer.

Queries are every time another dns server hits dnsmadeeasy's servers for a request. A hit doesn't necessary mean a hit to your allowed queries. Some ISPs cache records and refresh at the end of TTL....

Those are VPS servers going down. They tend to be more stressed than reseller and hosting accounts. I have never seen a reseller or hosting account go down at powervps.
