Is anyone aware of a clone of phpManager, or something like it to start off with that doesn't cost money? Is there anything that you could recommend for a form for billing?

Like SWR said, If you can't afford it or aren't willing to invest, then you might not want to be in this business. However, you might want to check out the "Running a Hosting Biz" forum and pick up the order form that alpha gave everyone.
