I am looking for an extremely cheap Windows host where I can host a small application (less than 10MB) and a small MSSQL database (less than 1MB). I realize that you "get what you pay for", but I'm willing to settle in this case as this is an application that only gets accessed a couple of times a month!

Any recommendations?

What are your requirements in terms of disk space/bandwidth? Do you have any preference for control panel?

My requirements for disk space would be about 15MB and my bandwidth requirements would be little to nothing. 1GB bandwidth would probably be more than sufficient. I don't have any requirements as far as control panel are concerned, I just need to be able to upload the ASP scripts and set up the MSSQL database.

At that level, barely if at all. Given how small your needs are I think you'd be best contacting individual hosts and seeing if they can put anything together for you as with the vast majority you'll be paying for a lot that you don't need.

What's very cheap? $5/mo? Under $5/mo?

Preferably I'd like to pay a small yearly fee of like, $15.00. I think that's reasonable given my requirements.

That's completely unrealistic - I'd rather recommend you to host on your local machine since I'm not aware of any providers with $15/m Windows hosting.

Not actually - it is important to note that web hosting is just not about disk space and bandwidth. There are lot of other factors - disk quality (SATA / 15K SCSI), bandwidth connectivity (10Mbps / 100 Mbps / 1 Gbps), Control Panel software, Mail software and not to forget Licensing cost of Windows!

With those kind of requirements I think VPS would be an overkill.

In my experience any shared windows hosting package will be sufficient.

"BrandonDC, by the sound of it, you need hosting with a database and you can get that for a few pounds / month. But don't sacrifice good support in favour of the cheapest package. In fact, for that few pounds / month, expect to get telephone and live web chat support. If not, look for an other hosting company.
