I was wondering if with abuse and violations of the AUP should you use a template email and how should it be formatted when information customers of their violation of a companies AUP or TOS; additionally how stern should you be on repeat offenders (people whom constantly install things like nulled IPB ect)

Violations come in different shapes and forms, using a template doesn't always work. I like to personally write them an email and notify what happened, what's going to happen, and who may be responsible.

And dealing with repeat offenders should be the same here as you would deal with a criminal. The second time around the punishment is always harsher. But depending on the circumstances, the person may not even deserve a second chance. SO you'd have to look at that as well. You have to treat each case individually because I doubt any two cases will be similar in every possible way.
