I am Jason and I am currently looking for a host which satisfies the following:

1) Windows hosting with support for MySQL - only using one database
2) ADO.Net connectors preinstalled (to connect my website to the DB)

Most hosting providers doesn't seem to mention if they support ado.net connectors.

Price is not so much of an issue as long as reliability and support is good. I am willing to pay up to $40/mo at the launch and more as my website grows, if it grows.

This sounds to be much reasonable and most Windows hosts supporting MySQL shouldn't have issues with satisfying your requirements. Typically, you should be a good candidate for shared hosting. However your budget is decent enough for VPS as well. Hence, if you are looking for getting total control over your hosting environment, you can consider VPS as well.

Shouldn't be a problem. There are a couple additional things you may want to consider. You sound like someone who will want remote access from local pc. Make sure (shared) host allows that --- for example connecting with VS mySQL plugin or a mySQL admin application.

You could get VPS hosting solution and install what you want on the server. Where are located your customers - US, Europe...The location is also important when you choose hosting provider.

With your budget you could go for a vps option which in my opinion has a lot of advantages on a shared one.
