Really cool setup.
The only problem was with the demo, I couldn't figure out how to make it notify you (as an operator) when someone comes to chat. I had to hit refresh to see it.
Did I miss something?

I had the same problem. Looks like your scrip may still have some bugs to be worked out...

Also, does this system have to possibility to initiate a chat with somebody browsing your site?

That would be a neat feature to have. I'll definitely buy a product with that feature included. By the way, Live Operator looks like a great product, so I'll definitely be trying it out.

If you have a problem with refresh or sound, try to update to IE5+ or make sure your media player is set to read .wav. Make sure to click on open pager when you'r in the admin panel.
