I've tried working with several scripts that upload an image to a folder, sends the image url to a MySQL DB, also on the same page a user can type in information regarding the image (location, comments etc) which is then sent to the same MYSQL DB.

Can anyone help me out here, point me to a good script that I can modify etc.

I don't have a whole lot lined up this morning and thats a 5 minute script, so if you like I can write the PHP/SQL for you if you write the HTML...or I can walk you through how to do it yourself over IM.
