I believe cd=Democratic Republic of the Congo

and sh=St. Helena

at least according to this reference:

List of .cd registrars: http://www.cd/cgi-bin/registrars.cgi?language=en

I believe easyspace.com provide .sh for £50, although I would not recommend them.

Q: Why would someone want to register a .cd or .sh domain name in the first place if he or she does not have any interest tied to those territories?

A: Perhaps it's because CD (Compact Disc) is a very large market which incorporate CD-R, CD-RW, CD-Rom, DVD, etc.; and SH could probably mean Shanghai (China) where ICANN's meeting is being held. :D

By the way, if you want to take a peep at ICANN's Shanghai meeting webcast, here it is:

Just for fun really

Its a bit expensive to become registrars for .sh and .ac (same company)
Most registrars join to get .ac

.cd is very difficult.
You need to use shell scripts to do the registrations.
