who takes the lowest percentage of the "profit" or amount, and lets you brand in the billing form on your site?

I use http://sfcommerce.com They don't have any rates on their site though. Email me if you want to know more.

I use Revecom,

No monthly fee
$49 setup fee
3.95% for CC orders 4% for checks
$1 authorization fee per transaction
$2 for voice authorization
