How much do accredited ICANN registrars buy .name domains for? Pricing seems to be varied.
first.last.name seems to be around $6, while the first@last.name seems to cost $12.
Can anyone confirm what the charges are?

You are substantially correct (as long as you add the mail forwarding costs to the domain costs for the bundle).
The pricing is so varied however due to the much lower volumes and the need to recover costs of integration with the registry, etc.
You can find .name domains for salt at retail for $15 if you shop, but you'll generally not find them discounted much since there is very little demand.

Due to the registry charging $12 for forename@lastname.name email forwarding, do the registry take care of email forwarding?
So in total, the registry charges $18 for a web site and email registration.

Yes, this forwarding is done at the registry, so the domain and mail forwarding costs become the cost of the "email bundle" from the registry.
