Personally, I searched for a designer when I first started my own website back in 2002. I came here (as a newbie) and searched for days for a designer. Not just the new posts, but hours digging through old posts and looking at portfolios.
Since it was 6 years ago, I don't remember if I posted asking for a template or just found them but I met Anthony somehow. Through all of the searching - his portfolio impressed me the most and I have never used another designer since.
FutureAdvanced.com is my designer until I die or he tells me to stop coming to him lol. Anthony is a great person and down-to-earth too... he has cut me deals when funds were low and even bartered my services for design updates. With being a great designer, he is a great person as well.
Being my personal friend and designer kind of makes me automatically say those things but if he weren't as great as I claim - I wouldn't still go back to him.http://www.futureadvanced.com - His website, with portfolio.

Haven't worked with any big devs and now that I picked up design as a hobby I haven't needed one in a while but San Antonio Web Design aka Boss Creative does really good work albit a bit too Flash-y for my tastes (though some of their Flex driven backend stuff they do is amazing).
Initially heard about them after seeing their work on a top design showcase website and then got to talk to one of their designers on a pro bono project and he left an impression when I had a chance to talk to him.
