I believe a registrar has breached the requirements outlined by ICANN to be a .com registrar.

Does someone have the necessary details that outline:
a) the service contract with ICANN that puts clearly what they are required to do?
b) the procedure for getting ICANN involved to punish this registrar for none service once I've confirmed they have violated the service agreement?

Have you visited http://www.icann.org/en/resources/

Specifically:

http://www.icann.org/en/registries/

More specifically:

http://www.icann.org/en/registries/agreements.htm

And even more specifically:

http://www.icann.org/en/tlds/agreements/com/

Good luck.

And this:

If by service agreement you mean yours and the registrar's, and not the registrar's towards ICANN, then I highly doubt ICANN will help.

ICANN won't do anything about any complaint you have with your registrar.
