Does anyone know of an easy to use AJAX driven form builder, which can save the captured form data to MySQL, and email it to a client. If it's Open Source, even better, cause then I can modify it and add code to it for our usage. I found Machform: http://www.appnitro.com/machform/ - which will cost me $39, but I don't quite know if I can add / modify the code to our liking.

It says that it comes with full source code, so you should be able to change it to your liking.

I already spoke to them, and the problem I have with it, is that I can't change the template to suit my own site, I can only changes the colors a bit, which is why I'm looking for something else.

And our end users won't be able to create the forms, without having to the HTML in order to publish it on the website.
