Do you need someone to do it? If so, I have seen 500-800 dollars.

Then there're many companies specializing in coding services like this one (http://w3-markup.com), or this (http://www.xhtmlized.com), or this (http://www.psd2html.com).. But they charge a bit more.

Depends, in person I charge around $600 but on DP and other places I do work for as little as $100, the DP and SP work is often much simpler.
