Hey guys I am looking for a provider who can provide the following:
VOIP (in/out) for business (with all standard features)
Managed PBX (Asterisk or something) must be fully managed, with the ability to add multiple numbers/auto attendants.
Full Fax in/out (that actually works properly).
I have looked at places like ringcentral, kall8, and aptela. Which I came to the conclusion that most people do not offer a fully managed solution, or all of the features we need.

I don't have any experience with their VoIP+PBX solutions, but Mailstreet (http://mailstreet.com/voice/) has some.

I've recently found Aretta after looking for something similar as you are. I've used RingCentral in the past, but their new VoIP implementation is really lacking, but aside from that, RingCentral was great.

also, for full fax in and out, is that just the .38 passthrough, or is that like fax to email and email to fax type apps, or both?

I currently use Junction Networks - both their SIP service and their hosted PBX. It works extremely well.. I don't currently use them for Fax but rather outsource that to TrustFax (which is a subsidiary of Comodo). They send you a PDF version of all incoming faxes. You can also send out faxes, they convert PDFs and other uploaded files and fax them automagically.

Regarding fax... It is not easy to manage inside VOIP.
I would recommend you to use a service like www.efax.com
