I'm in the UK, and I have both a UK landline number and a UK cellphone number.

What I'm looking for is a 'follow me'-type service where I can have a number that I can change to route to my cellphone or my landline.
-BUT-
I'd ideally like 3 (yup, three!) numbers that can follow me:
UK landline no. (don't care about the area code)
US landline no. (don't care about the area code)
US toll-free

I understand the toll-free bit will probably cost more, and the routing to my UK cellphone will be pricey too - but who can offer such a service?

I figure Skype can - but is SkypeIn reliable enough?
Are there any other providers?

We use Gradwell to provide a UK 0800 (free) number which redirects to the on call staff member. And we have Skype for a UK 020 (London) number (for historical reasons - Gradwell can provide this 020 number too). Using the Skype forwarding, we forward to the 0800 number Gradwell provide which means we can get any number of Skype numbers worldwide. I have a feeling Gradwell would be able to provide international numbers too, but you'd need to check that with them.
