What are other hosting companies using to provide fail-over and high-availability of customers sites? In shared and managed service offerings? I know hardware load balancers can do it, but they are expensive. Are there any other alternatives?

Who uses clusters and who uses load balancers? And what about software load balancing clusters like Microsoft NLB?

consider Cisco CSS web switch... nowadays, they cost less and are rather reliable. There are other companies that offer load balancing/failover as well and they charge less than Cisco..
ps. why don't you look and bid on some LB devices on Ebay.. they are pretty cheap there - just watch out for fraud!
