Anyone know of a simple PHP/MySQL knowledge base?
This will be for internal use only so I can keep track of mistakes made and solutions found. I thought of installing a forum and then stripping it down to remove all the usless features like smilies and just use that. But I wanted to see if there is a better solution before I start on another project.

Personally, any KB software would be fine, that someone recommends.

faq-U is good. It's a bit old now, and the new version never did seem to appear but it's still a handy bit of software

I just need something simple and free, considering it's not a must have but would be nice to use

If you get to the point where you can afford to purchase a Kbase you should look into Kbase by http://scripts.tlcwe.com/ . I use it on my site and it is very easy to use and very easy to customize.
