From what I can see, you only get free setup if you pay for 12 months of hosting in advance. Its 69.99 setup for monthly hosting...

daniel is right: The longer you pay in advance, the less the setup fee will be. The maximum setup fee is for a month-to-month-contract (69,99 Euros one-time).
