Hardware upgrades have a $9.99 setup fee. If you add an additional hard drive, you will have to pay a $9.99 setup fee.

OS reloads are free(limit to 5 reloads a month). If you reload your OS more than 5 times a month, you will have to pay $9.99 setup fee for the OS reload.

Changes to bandwidth have no setup fee associated as there are no hardware changes in this case.
