Hi there, fellow mates i have some trouble with my php form & would like to seek help from forumers over here to help to solve the issue.

The above link is the php form that i need help with, i have stated all the fields but it seems theres still missing fields when i already fill up all the infos.

Check that the field names match exactly.
Rather than give us the script, we'd need to see your form, and any changes you may have made to the script.
