24/7/365 support through phone and live chat with very fast response times?

The charge should be monthly per customer would be ideal..

I don't have experience with any of them, but if memory serves correctly they all offer end user support in one form or another. Or you could try hiring your own outsourced support team.

tripodsupport.com is supposed to be very good. I have not tried they myself however it may be worth checking them out.
