Well, that loads, and I can submit the empty form, no error. Can you be a little more specific about what you feel is wrong with it?
- No email received
- Errors out
- something else...

I managed to fill it out also.

If it is not working for you directly, such as you are not receiving the mail, can you check your mail server is not bouncing it? Did you do any changes?

do a tail -f /var/log/maillog and get back with what its written over there.
