If I wanted to list some of my clients on my web site, etc... Should I get them to sign something saying I have there permission or should just asking them and them saying yes be enough?

I would say yes you would most likely need some type of permission. Sometimes potential customers ask for a url of a customer you have to test speed and so on which I usually go along with but I dont know about listing your customers on your site.. Your know what they say better safe than sorry..

Unless your TOS and your privacy policy state specifically that you will do so.

Also, I believe a poor practice as I have seen a customer get mad at a host and then target all the customers.

I would say getting just a "yes, that is ok" would be enough.

Here's another idea - you can tell them you are offering it as a new service to them. Free advertising and such. Then they will not only say it is ok, they will thank you for doing it!

Always get permission when using some one elses data/information... its better to be on the safer side ;)
