Anyone know where I can get a script like what proxify.com has?

That site is a front end for phpproxy (https://sourceforge.net/projects/phpproxy/).

Or cgi proxy (http://www.jmarshall.com/tools/cgiproxy/)

Now it's use CGIproxy as state in proxy.org

but now proxify is charge you for SSL and video site

Try another like Delprox.com or other that have this features for free.
