What is the best way?
A one-time setup fee, or a monthly recurring fee? (Or perhaps both?)
Just curious to know what others are doing..

If it's just domain parking with a simple "i'm parked at xxx" page i'd pay $5 tops.

I think free will be best. Put a small ad on the page, it helps alot.

Someone actually registered WTCdeathcount.com with GoDaddy and Reader's Digest reported that when they went to the horribly named site, there was an ad for Go Daddy Software. So the Reader's Digest made it look "like Go Daddy Software is purposely using that terrible domain name to advertise its products."
