Help me please.

I am on the look out for some reliable, tested host, which is also not VERY expensive.

I need the following features:

PHP 4.1.0 or above
MySQL 3.23.47 or above
PHP should not be running in safe mode
PHP functions like md5, mkdir, and mail should be enabled.
PHP Extensions like zlib, gd, mcrypt should be enabled
Latest phpmyadmin installation provided
Enough rights on the bought account to allow visitors to upload files on the server
Enough rights to create new folder with custom rights.
.htaccess and subdomains would be nice, though optional.

I would be obliged if you can suggest me any good host matching these criteria.
