CF 5.0 (preferrably)
SQL Server 2000
6GB+ bandwidth
50MB disk space
100MB Database
SMTP server

I have searched the forums and found out the following hosts:

Now its decision time -- I can afford $20 per month

A few comments:

1) MediaTemple.net seems to be a Linux-only shared hosting service so that knocks out your CF requirement.

2) You should check out what others have to say about GearHost and its predecessor companies elsewhere on this Web Hosting Forum.

3) You might want to check out the offerings of www.uplinkearth.com.
